Medicinia – Hospital Care

Medicinia‘s Clinical Relationship Management (CRM) platform enables providers and payors to have a continued and semi-automated relationship with their patients. It involves a combination of automated (robot-based) workflows with the participation of human navigators, usually nurses. In this pilot, HospitalCare deployed the solution to assist their home care team in various areas, such as caretaker turnover, call shifts, education, alignment between family members, clinical decompensation, direct communication with the attending nurse, and more.
